What's the name of the part the triangular shaped plate that's used to relocate the rear brake reservior near the right side passenger pegs ???

Vance & Hines part 333-P. Supposedly costs less than 20 bucks. This should be a link to the installation manual. There's a phone number you can call on it where you can order the part.
