If this warranty is valid on the day you buy your tire then you are covered. Use one form for each tire purchased. Fill out the form and keep it. No need to send it to Avon. Write name of store where you bought the tire and their phone number in the 'dealer stamp' box (nobody uses stamps anymore). If you have trouble, take the filled-in form to any Avon tire dealer; ignore the Avon North American screen saying go back to where purchased it is not a requirement of the warranty per this form and Avon UK. For other questions, read both sides of the warranty form. See actual form attached below.
